Open Rhinoplasty
An Open Rhinoplasty includes the same two incisions, but with one additional incision connecting the two, across the columella. That final connecting incision is called a trans-columellar incision, and it allows Dr. Papanicolaou to open the skin of the nose.
With this technique, Dr. Papanicolaou will elevate the skin to assess all of the various nasal tissues and structures, then, he will have full access to perform corrections such as removing excess tissue, reshaping the cartilage and nasal bones, or placing cartilage grafts. Once these corrections are finished, he will reposition the skin and suture it back up for the healing process.
An Open Rhinoplasty will result in a scar at the incision site, however, this is usually well concealed. As time passes, the scar will fade and remain minimally visible.
Closed Rhinoplasty
With a Closed Rhinoplasty, also called Endonasal Rhinoplasty, any incisions and nose tip reshaping are done within the nostrils. Closed rhinoplasty techniques are generally used when the patient desires minor corrections and reshaping of the nasal tip.
Dr. Papanicolaou begins your procedure by making a small incision inside each nostril. From there, he will make nasal corrections as needed, such as sculpting the cartilage and nasal bones, eliminating excess tissue, or adjusting the shape/size of the nostrils. When complete, Dr. Papanicolaou will use dissolvable sutures to close the incisions.
There is no visible scarring after a Closed Rhinoplasty, however, since access is limited with this technique, it cannot be recommended for all patients.

Nonetheless, this minimally invasive option (a.k.a. liquid rhinoplasty) can work wonders for some patients with small contour deformities. Using fillers similar to the products used for cheek or lips augmentation, dorsal depressions or flat nasal tips can be treated to smooth the contour or improve tip projection. While the fillers are only temporary, they can be a great option for someone not ready for surgery or as a quick fix for an upcoming event (i.e. wedding, graduation, reunion, etc.).
